New Patients
What to Expect?
We’re excited to welcome you to our practice and make your first visit as easy as possible.
Before your appointment:
​You'll receive digital intake forms to complete at your convenience. We ask that you complete it within 48 hours of your visit.

Please bring copy of your drivers license and Insurance card

If you’ve seen another eye care provider, feel free to bring any prior records or have them sent over prior to your appointment.
At your visit:

Frequently Asked Questions
Common symptoms include dryness, burning, redness, irritation, fluctuating vision, excessive tearing, and contact lens discomfort. Many patients are surprised to learn that watery eyes can actually be a sign of dryness. An evaluation is the best way to determine the underlying cause and create the right treatment plan.
We are a specialty clinic focused exclusively on dry eye and ocular surface disease. Using advanced diagnostic technology, we identify the root cause of your symptoms and not just treat them temporarily. Our approach is highly personalized, combining in-office treatments, at-home care, and ongoing follow-up.
We are in-network with several medical insurance plans and are able to bill for exams and certain services when applicable. However, as a specialty dry eye clinic, some of our advanced diagnostics and treatments are not covered by insurance.
Our team will review all coverage, costs, and options with you prior to your visit so there are no surprise
We work closely with referring doctors and your primary eye care provider to ensure continuity of care. After your visit, we can share findings, treatment plans, and progress updates so your entire care team stays aligned.
